It is essential that we are notified of changes to your name, address, home or mobile telephone number. There are two ways of notifying us of any changes:

• Online:
  Use our online form to tell us about the change.
• In Person or by Telephone:
  Please call in for a form or telephone the Registrations Administrator.

If you move out of the practice area you should not assume the Practice can continue to look after you. You will need to find a doctor in your new area.  There are instructions when you move to a new area on the back of your NHS medical card.
