Appointments
The appointment system operates both for your convenience and to
aid the Doctors. An appointment is for one person only. You may consult any of
the Doctors in the practice, not just the one with whom you are registered. You
may book the first hour of the morning up to 9.50am for the next 3 working days.
The rest of the morning and all evening appointments are booked on the day at
8.45am. Patients are kindly requested not come down to the surgery but to
telephone at 8.45 to book an appointment on the day.
The Doctors also offer some telephone consultations each day.

TELEPHONE ADVICE – 020 8764 0407
Sometimes you may want advice on a health problem or
medication and simply wish to talk to a Doctor. The duty Doctor is available
after 2.00pm each day.
It is very helpful to let the receptionist know about whom or
what it is concerning, so that any records can be made available if required.

HOME VISITS
Please do not ask the Doctor to call unless the patient is
genuinely too ill to come to the surgery. Remember that the Doctor can see
approximately four patients in surgery during the time it takes to do one home
visit and it is at the Doctor’s discretion as to whether a home visit is
necessary. If the patient has a temperature or rash, coming to the surgery will
not endanger others, but please inform the receptionist on arrival. When you do
require a home visit, please try to telephone before 10.00am on the same day, as
this enables the Doctor to plan calls and save time. When you request a visit,
the receptionist will ask for details of the patient’s name, address, age and
telephone number and the reason for the visit.

OUT OF HOURS EMERGENCIES

5.00 One is situated in the car park at St George’s Hospital and another at 45
High Street Croydon. These Centres are open Monday to Friday 7.00am to 10.00pm.

repeated if you contact the surgery out of hours.

also available for any medical problems that you may wish to discuss.
The telephone number for NHS Direct is 0845 4647.

REPEAT PRESCRIPTIONS
Please allow two full working days for prescriptions to be
processed and signed before collecting. Leave your computer counterfoil in the
box provided (at the gate, in the entrance porch or at reception) ticking the
items you require. Postal requests must be accompanied by a stamped addressed
envelope. To avoid errors, telephone request cannot be accepted except for
housebound patients. We like to see people who are on repeat prescriptions from
time to time. We may put a reminder on your prescription counterfoil about this.

TEST RESULTS
In order to maintain confidentiality, laboratory results and
x-ray results will only be given to patients themselves or to parents of minors
if appropriate. When enquiring about a result, please telephone between 10.00am
and 5.00pm when results will generally have been received and seen by the
Doctors.

New Patients
All newly registering patients will be asked to complete a
form and wait for their health checks to be done. This gives us an opportunity
to have some background to your health before your notes arrive from your
previous Doctor. Children under the age of 5 years do not require a health check
but parents/guardians should be able to provide immunisation details.
The times for Registering with the Practice are Monday to
Friday 09.00am to 5.00pm (please remember the surgery is closed from 12.30 on
Wednesday).

up to the first week can only be provided as a private certificate for which

heart or chest complaints, diabetics, elderly, or those who work in institutions

by the primary care team. In particular, care is focused on the physical health
